I am a Mental Health Rehabilitation Therapist/Clinician, a Credentialed Mental Health Nurse, and a Registered Nurse.

For this support item I provide collaborative metal health consultation, care-plan support, referrals, and communication with you and your other care and support providers. I am available for consultation face to face, by phone, or email as needed.

Individual Assessment & Support by a Nurse (Registered Nurse) Item Ref No: 15_036_0114_1_3 $95.77p/h

This Support Item is provided to help you to manage your medications and includes:

Support with self-management

Regular medication administration/compliance support incl. injections

Medication & script arrangements and collection

Information about medications for you and your supports

Education for you and your supports about your diagnosis/illness and management 

Attendance at relevant health/mental health appointments e.g. GP, Psychiatrist
